Who Qualifies
Crossroads Clinic Volunteers In Medicine offers free primary care medical services to Missouri residents who meet the following criteria:
- 18-64 years of age
- Reside in Western St. Charles, Lincoln, Warren or Montgomery Counties
- Have NO medical health insurance including disability, Medicaid, Medicare, etc.
- Household income status at 200% or less of the Federal poverty level.
How to Get an Appointment
If you fit the guidelines, then you can download a New Patient Registration Form here, request one by phone or pick one up at the clinic on Tuesday or Thursday between 9:00am – 1:00pm.
After completing the application, please CALL our office at 636.561.3133 to find out what types of income documents you will need to provide.
After gathering the needed income documents, you may a) mail them or b) submit the application in person to our office.
Once the application and requested income documents are received and your application has been reviewed to ensure all criteria are met, the applicant will be called and an appointment scheduled.
Exceptions to this process can be determined based upon the urgency of the patient’s situation.
