Patient Rights

We, the staff and the board members of Crossroads Clinic (Volunteers in Medicine) are dedicated to ensuring that you will receive quality care and are treated in a professional manner.

  • All clients being treated with respect and consideration at all times.
  • All information pertaining to each client’s diagnosis and treatment is confidential.
  • All clients shall have access to complete and current information pertaining to his/her condition.

 

Client Responsibilities

Clients will strive to keep appointments for the clinic and any specialists referred to.  In case one needs to cancel, he/she will notify the clinic/specialist at least 24 hours prior to the set appointment.

Clients shall complete the “Fee Determination Information” form and bring proof of total family income on the first visit to the clinic.

Patient Privacy

At Crossroads Clinic Volunteers in Medicine, your privacy is very important to us. We treat your personal information with care and we have policies and practices to ensure your information is kept safe.

We will only share your information when needed to facilitate your care. This could include communication with other providers, such as doctors, pharmacists and nurses.

We will ask your permission to share your information for any other reasons and will only do so at your request and approval.
